Athletic Student Certifications
Students must complete the below certifications in order to be able to participate in athletic activities at Gateway High School. To access each certification, click on the below links and follow the on-screen directions. All certifications must be completed online prior to being eligible to participate in athletic activities. the following is how to upload these certifications to your Athletic Clearance account:
once the steps above are completed in your Athletic Clearance profile, you will then access the Files tab. This is the section where you will be uploading all your signed documents into their appropriate areas. These will need to be in PDF form. There is a free app for phones called Genius Scan. You can download this, and it will turn any document into a single or multi page PDF file. It is very user friendly, and you can then email yourself the PDFs to upload. You will need to upload the following documents…
- EL2 FHSAA physical form - Please be sure that the forms are signed, stamped and dated by a Physician where indicted:
- YOU MUST UPLOAD THE MOST UP TO DATE EL2 FORM: CURRENT FORM IS 2/2026. THE DATE CAN BE FOUND IN THE UPPER RIGHT HAND CORNER OF THE EL2 PACKET
- EL1 (EKG or ECG Baseline) – Required for ALL NEW Students in the School District and any incoming 9th grader and MUST be read by a cardiologist
- FHSAA Concussion Video Certificate:

- EL7 IS FOR HOME SCHOOLED STUDENTS ONLY- You must be assigned to Gateway High School by the School District of Lee County to participate in sports.
