DIRECTIONS TO COMPLETING GATEWAY HIGH ATHLETIC CLEARANCE

1. Go to www.AthleticClearance.com and choose the state of Florida. Create an account with a valid email address and a password that you will REMEMBER.
2. Select the 26-27 school year, choose Gateway HS in Fort Myers, Florida, and then select ALL the sport (s) you will be playing or interested in. (it is easier to select them all now instead of reapplying.)
3. Complete all the fields in the student tab. Click Save & Continue once you have completed EVERY field.
4. Complete ALL fields in Parent/ Guardian tab. Click Save & Continue at the bottom once you have completed EVERY field. Click NO at the bottom for Student Recruiting Services or you will receive a lot of junk emails.
5. Answer ALL the questions in the medical tab. Please provide the needed proof of Health Insurance.  At the bottom, click on the pinkish box to open and print the two-page FHSAA Physical form you will need signed by your physician. Click Save & Continue once you have finished.
6. The next tab is Signatures. In this tab you will do several things.

• The student must electronically sign for each of the three EL3 forms.
• You will need to print out each of those forms as well as to physically sign and then upload them back into the system later in this process.
• Once you have completed those steps, click Save & Continue at the bottom.
• You will then be prompted to sign each of the three forms again plus one additional electronically with the PARENT name this time. Once you complete that, click Save & Continue at the bottom. 

Athletic Student Certifications

Students must complete the below certifications in order to be able to participate in athletic activities at Gateway High School.  To access each certification, click on the below links and follow the on-screen directions. All certifications must be completed online prior to being eligible to participate in athletic activities. the following is how to upload these certifications to your Athletic Clearance account:

once the steps above are completed in your Athletic Clearance profile, you will then access the Files tab. This is the section where you will be uploading all your signed documents into their appropriate areas. These will need to be in PDF form. There is a free app for phones called Genius Scan. You can download this, and it will turn any document into a single or multi page PDF file. It is very user friendly, and you can then email yourself the PDFs to upload. You will need to upload the following documents…

• EL2 FHSAA physical form - Please be sure that the forms are signed, stamped and dated by a Physician where indicted:
  • YOU MUST UPLOAD THE MOST UP TO DATE EL2 FORM: CURRENT FORM IS 2/2026. THE DATE CAN BE FOUND IN THE UPPER RIGHT HAND CORNER OF THE EL2 PACKET
• EL1 (EKG or ECG Baseline) – Required for ALL NEW Students in the School District and any incoming 9^th grader and MUST be read by a cardiologist

• EL7 IS FOR HOME SCHOOLED STUDENTS ONLY- You must be assigned to Gateway High School by the School District of Lee County to participate in sports.
